What is causing my Kia Rio to make a loud grinding noise while braking?

I have noticed that my Kia Rio is making a loud grinding noise every time I apply the brakes. It seems to be coming from the front of the car and it has been getting progressively worse. What could be the cause of this issue and what can I do to fix it?
