What should I do if my Kia Soul is experiencing rough shifting?

I've noticed that when I'm driving my Kia Soul, the shifting feels very rough and clunky. It's especially bad when I'm going from a complete stop to accelerating. What could be causing this issue and what can I do to fix it?
