What should I do if my Kia Sportage is experiencing rough shifting?

Lately, my Kia Sportage's transmission has been shifting roughly, causing a jerking sensation. Sometimes it happens when accelerating, other times when decelerating. It's becoming increasingly concerning and I'm not sure what to do. Any advice?
